Chapter 12 side quest badge?

I was wondering if anyone out there has received the badge for the chapter 12 side quests? I have searched every corner, dungeon, shop and house in the Subata skyway, but I still cannot find any missing quests.

I am also missing the Chapter 12 side quest badge, along with the one for Chapter 8, and Chapter 3 (which we were told is bugged).

There are other posts on this board regarding the chapter badge as well.
